Documents

The following is a list of documents that may be necessary to provide the United States Citizenship and Immigration Services. Please consult with an attorney to determine which documents are required in your case.
  1. Three (3) photographs of Applicant and two (2) photographs of Petitioner.
  2. Medical Examination of Applicant (sealed in an envelope by the doctor).
  3. Completed Questionnaire.
  4. Birth Certificates of Applicant and Petitioner (must be translated into English).
  5. Copy of Alien Registration Card/Naturalization Certificate of Petitioner (if applicable).
  6. Military Documents (if applicable).
  7. Police Records (if applicable).
  8. Marriage Certificate (if applicable).
  9. Divorce Decree(s) (if applicable).
  10. Copy of Applicant's Passport and I-94 Card (front and back).
  11. Evidence of your Marriage (including but not limited to):
a.
 Birth Certificates of any children;
b.
 Joint Lease or Property Deed;
c.
 Joint Bank Account Statement;
d.
 Insurance Policies;
e.
 Affidavits/Letters from acquaintances (must be notarized);
f.
 Bills/Receipts issued in joint name;
g.
 Family Photographs;
h.
 Any documentation showing the existence of the marriage.

12. Evidence of Support (for Petitioner and Joint Sponsor if necessary):

a.
 Complete Federal Tax Returns for the last three (3) years;
b.
 Letter verifying employment on company letterhead, indicating salary, job duties and dates of employment, executed less than 30 days prior to filing;
c.
 Copies of W-2 Forms/ Pay stubs;
d.
 Evidence of Permanent Resident or Citizenship Status);
e.
 Evidence of other assets (if necessary)

13. Degree(s) and Degree Evaluation (if applicable)
14. Letter from previous employer(s) on company letterhead indicating job title, job duties and dates of employment.
15. Evidence of lawful maintenance of prior immigration status.
